Methodology & Background The Patient Navigation Center served 2,176 unique individuals during its first six months of operation Patient Navigators assisted patients through 5,252 interactions, comprising phone calls, emails, texts, website live chats, and Skype calls. The Patient Navigation Center caseload has grown 328% since opening in February 2016 Health education inquiries have grown 315% compared to the same time period during the previous year (2015 )

Stories from the Navigation Center Emily Newly diagnosed with guttate psoriasis Under a lot of stress In working with her Navigator, we were able to: Connect her with a new doctor closer to home for her phototherapy appointments C oached her through ways of making her phototherapy trips easier (travel with her dog, shopping, finding a park to walk, etc.). Provided her with disease management and treatment information C onnected her with a volunteer in Psoriasis One to One

Stories from the Navigation Center Beth Contacted NPF seeking help in finding a rheumatologist Has had significant inflammation and pain in her connective tissue and some nail changes in finger and toe nails. Was diagnosed several years ago with PsA , but recently had the diagnosis removed and treatment suspended In working with her Navigator, wee were able to: Help her locate a new rheumatologist who reinstated her psoriatic arthritis diagnosis and created a long-term treatment plan for her She is very happy with her new care team   and the level of attention they provide to her  

Stories from the Navigation Center Sally Parent of a teen with psoriasis Unsatisfied with current treatment Suspected psoriatic arthritis In working with her Navigator, we were able to: Connect her with additional specialists in her area Provide information about other treatment options and combination treatment plans Provide her with information about how to get involved with local events in her area to meet other teens living with psoriasis

Stories from the Navigation Center John Has severe genital psoriasis Feeling isolated, discouraged with treatment options Contacted the Navigation Center for support In working with his Navigator, we were able to: Connect him with another volunteer living with genital psoriasis through the One to One support program Help prepare him to discuss other treatment options with his dermatologist Coached him through stress management strategies and positive ways to cope

Stories from the Navigation Center Clara Has had severe psoriasis all of her life Negative experiences with health care providers in the past have led to fear and ambivalence about seeking treatment    Currently not treating or seeing a health care provider In working with her Navigator, we were able to: Utilize coaching and motivational interviewing strategies to help Clara overcome her anxiety about seeking medical care Locate a qualified specialist in her area Clara is now seeing a dermatologist for the first time in many years and is optimistic about treatment
