Maria Cristina Mansur, M.D. Maria Cristina Mansur, M.D.Medical Director, Department ofDermatology Centro Cient’fico deProfessor, Universidade Federal de Juiz de ForaÑMG, Brazil Introduction Ephelides, or freckles, are epidermal-ed lesions common in exposureeas. Freckles are often difficult to treat, as patients tend to refreckle with additionaly respond hed laser treatments; but consid-ering that longer wavelength lasers for hairget epidermal melanin, it hat hair removal lasers wouldalso be effective in treating these superficial Method Case 1 Ð Patient skin type I Fitzpatrick witheckles on the face. Two laser treatmentswere made at monthly intervals, using a 12 mm spot size at a fluence 25 J/cm55 nm wavelength, 3 ms pulse durations, Ð Patient skin type II Fitzpatrick with freckles on the upper back. Previousattempts to treat with cryotherapy producedunsatisfactory results. Three laser treat-ments were made at monthly intervals, using a 12 mm spot size at a fluence In both cases, the GentleLASE alexandritelaser from Candela was used with the DCD. Results The ephelides (freckles) darkened in colorely after treatment and scaled offeek, resulting in significant clinicalimprovement. The postapplication photoswing the laste instructed to use sunscreen protection (of at least xposure Discussion Immediate, ablative photothermolysis of ed lesions occurs with Q-Switchednanosecond pulsesnanosecond pulseswever, lasers with longer wavelengths are alsoeventually effective in treating these types obably because the target is more the melanocyte versus themelanosome, which is the Q-switchedlaserÕs target.Greater efficacy was observed using theGentleLASEÕs smaller spot sizes, as a laserÕsenergy is not introduced as deeply into skintissue as spot size decreases.
