What happened A Toyota pickup truck rolled over while coming from Hazeera to Bahja The vehicle was moving on Zauliya road towards Bahja when approx 23 km away from Bahja PDO camp while negotiating a curve on the graded road the driver lost control and the vehicle moved left to the upcoming lane The driver tried to regain control but the vehicle skidded and rolled over 3 . What happened?. An Auto-mechanic was removing the clutch disc from a Water tanker truck in P&E workshop with the help of his colleague. While doing this activity the clutch disc slipped and fell on his left hand middle finger tip causing a crush injury. He was given First aid at site and from hospital he was diagnosed with fracture in tip of the finger.. : LTI. What happened. ?. During process of Derrickman (DM) changeover, Floorman (FM) relieved DM and pulled & racked back a stand of pipe. DM was standing close to ladder with fall arrester attached in preparation to descend down the ladder. Driller lowered Top Drive System (TDS) which entangled and pulled the fall arrester line along with DM. DM fell (same level) at monkey board platform with left leg trapped between monkey board post & guide beam resulting in a fracture to his left ankle. . What . happened?. The flowline pipe was rested on the tripod and due to the stress relief the pipeline straightened and fallen on the permit holders legs as he was walking parallel to the line which resulted in fracture to his right leg. .
