Cost Category (DWSRF)The Drinking Water State Revolving Fund Project Information Form requires applicants to indicate the cost categories associated with the proposed project.Selection of Cost Categories will impact the availability of choices on the Estimated Costs table.23

Rating CriteriaThe rating criteria automatically guides the user based on Project Information Form type.Additional questions may be generated based upon the user’s answers.24

Estimated CostsAuthorized SignatureSignature of Entity’s Authorized Individual is required to be attached with Estimated Costs.For projects with costs over $100,000, a professional engineers signature and seal is required to be attached.30

Project Information Form SubmittalThe form must be electronically signed by the Entity’s Authorized Representative.Only an Applicant or user that has been assigned submittal capabilities may submit the form for review. 38
