HOPES Anjali Chacko and Hannah James Community Engaged Learning Eastern Virginia Medical School Norfolk VA Introduction HOPES Free Clinic is the only studentrun free clinic in Virginia and one barrier to effective patient care is the lack of standardization in patient notes Student clinicians often have never used Practice Fusion before leading to longer visits and not being able to see as many patients By introducing templates into Practice Fusion notes can be standardized and thus allow for less time spent writing notes as well as potentially aiding clinical decisionmaking In order to do this the first step was to determine what templates would be needed by analyzing which conditions were most frequently seen in each clinic .
