Select Paper or E-mail Statements $ 0.75 deducted from HSA balance each quarter if you select Paper statements
Add Beneficiaries Beneficiaries can be added (or changed) later, as well
Post-tax (personal) contributions Select an Optional box if you expect to make post-tax (personal) contributions directly to BNY Mellon (not via payroll)
Confirm your information Almost done… be sure to click Finish and Submit
