17. 172:02pm – Manufacturer AOn December 6th, Manufacturer A received notice from Business A. A customer provided information that two broiler houses in the same county reported multiple illnesses and deaths.

21. 219:07am – ConfirmedAll birds in the two broiler houses were being fed feed manufactured by Manufacturer A, which came from the previously identified suspect production runs.

24. 2410:28am – Feed Shipping InfoFeed from the suspect production runs were shipped to Business A and Business B.
25. 2512:12pm – FDAA consumer safety officer from the FDA District Office contacted Manufacturer A. The FDA advised Manufacturer A that a veterinary clinic had notified the District Office that a broiler house had suffered the illness or death of 40,000 chickens from what appeared to be aflatoxicosis.

32. 321:37pm – FDATwo consumer safety officers from the FDA District Office visited Manufacturer A to review bills of lading, testing records, customer complaint lists, production records, standard operating procedures, and training documents.
33. 331:59pm – Manufacturer AWe still cannot confirm where the feed from Run B was shipped. “Our corporate IT department reported ‘intermittent data anomalies’ from November 25th thru December 4th. This is impeding our ability to identify the actual destinations of the feed from Run B.”

44. 442:26pm – Manufacturer A & FDA27 complaints have been received of sick and dying chickens in the last week alone.
45. 452:52pm – FDAOfficers from the FDA District Office are scheduled to be at Manufacturer A at 10:00AM tomorrow.
46. 464:04pm – Additional TestingManufacturer A submitted additional retained samples from the five prior and two subsequent production runs of grain-containing products.
47. 475:00pm – Local NBC News“…deaths of thousands of chickens due to contaminated feed. Company A and Company B stores have removed all Manufacturer A products from their shelves. Anyone who has purchased any Manufacturer A products can return them for a refund.”

56. 5610:04am – InvestigatorsFDA & State Investigators returned to both Manufacturer A and Business A facilities. They expanded their review to include the food safety plan, with specifically focusing on hazard analysis.

68. 689:47am – Manufacturer A80 complaints have been reported that warrant further investigation. Each is associated with one or more poultry illnesses or deaths. No illnesses or deaths in other animals have surfaced.
69. 6911:11am – Manufacturer AManufacturer A has formally requested that the FDA and the Dept. of Agriculture share their aflatoxin testing results and list of consumer complaints to cross-check against customer complaints.
70. 701:05pm – CNN“Today, China refused multiple shipments of corn from the US citing the aflatoxin contamination incident in the eastern United States.”
