The Control of Hazardous Energy (Lockout-Tagout)

Report Errors to ManagementSlide2

CSP shall establish a program consisting of:Energy control proceduresEmployee training

Periodic inspectionsThis procedure for the control of hazardous energy shall comply with the OSHA 29 CFR 1910.147 and shall clearly outline:The scopePurposeAuthorizationRulesTechniques

Lockout-

Tagout

Procedure

Report Errors to ManagementSlide3

The procedure shall ensure that before any employee performs service or maintenance on equipment, the equipment be isolated when an injury could occur do to the following:Unexpected energizingUnexpected startup

Release of stored energy (pneumatic, mechanical, electric, chemical)Lockout -Tagout ProcedureReport Errors to ManagementSlide4

The procedure shall also define the means to enforce compliance as follows:A specific statement of the intended use of the procedureSpecific procedures for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy

Specific procedural steps for the placement, removal and transfer of lockout devices, and tagout devices and the responsibility for themSpecific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures

Lockout -

Tagout

Procedure

Report Errors to ManagementSlide5

Lockout and tagout devices shall be capable of withstanding the environment to which they are exposedTags shall not deteriorate when used in corrosive environments

Lockout and tagout devises shall be standardized within CSPLock out devices shall be substantial enough to prevent removal without the use of unusual techniques or excessive forceLockout -

Tagout

Devices

Report Errors to ManagementSlide6

Tagout devices, including their means of attachment, shall be substantial enough to prevent inadvertent or accidental removal. Tagout device attachment means shall be of a non-reusable type, attachable by hand, self-locking, and non-releasable with a minimum unlocking strength of no less than 50 pounds and having the general design and basic characteristics of being at least equivalent to a one-piece, all environment-tolerant nylon cable tie.

Lockout -Tagout Devices

Report Errors to ManagementSlide7

Lockout devices and tagout devices shall indicate the identity of the employee applying the device(s)Tagout devices shall warn against hazardous conditions if the machine or equipment is energized and shall include a legend such as the following: Do Not Start. Do Not Open. Do Not Close. Do Not Energize. Do Not Operate.

Report Errors to ManagementSlide11

The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of the standard are being followedThe periodic inspection shall be performed by an authorized employee other than the one utilizing the energy control procedure being inspected

Periodic InspectionReport Errors to ManagementSlide12

The periodic inspection shall be conducted to correct any deviations or inadequacies identified. The employer shall certify that the periodic inspections have been performed. The certification shall identify:The machine or equipment on which the energy control procedure was being utilized

The date of inspectionThe employees included in the inspectionThe person performing the inspectionPeriodic Inspection

Report Errors to ManagementSlide13

The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

Each affected employee shall be instructed in the purpose and use of the energy control procedureTraining and Communication

Report Errors to ManagementSlide14

Lockout: Process of preventing the flow of energyConsists of installing a lock, block, or chainRequires an Authorized Employee to apply/remove lock

Definitions: Lockout

Report Errors to ManagementSlide15

Tag Out: Process of placing a warning tag on the energy sourceThe tag acts as a warningThe tag must clearly state “Do not operate”The tag can be used when the equipment won’t accept lock

The tag must be applied/removed by an Authorized EmployeeDefinitions: Tag Out

Report Errors to ManagementSlide16

Authorized employee:A person who locks out or tags out machines before the servicing or maintenance of equipment can proceed.Affected employee: An employee who will do work on the equipment

An employee who works in the areaCannot help authorized employees service machineryDefinitions: Authorized and Affected employee

Report Errors to ManagementSlide20

The following steps apply to the lockout/Tag Out procedurePrepare for shutdownShutdownIsolate equipment from energy source

Apply Lock Out or Tag Out deviceRelease of stored energyVerify isolationLOTO: Steps

Report Errors to ManagementSlide21

1. Prepare for ShutdownKnow the type and magnitude of energy sourceKnow the hazards of the energy sourceKnow the methods or means to control it

2. ShutdownTurn off equipment using normal controlsUse orderly shutdownLOTO: Steps

Report Errors to ManagementSlide22

3. Isolate EquipmentIsolate equipment from energy sourcesSecure lock to energy isolating device

LOTO: Steps

Report Errors to ManagementSlide23

4. Notify affected employeesAttach lockout/

tagout devicesOnly authorized employee can attach or remove lockout and tag-out devicesDevices must be durableDevices must identify person who applied them

LOTO: Steps

Report Errors to ManagementSlide24

5. Release Stored EnergyRelieve stored energyVerify machine isolation from energy Relieve pressure in compressed air line

Add interference elements if applicableLOTO: Steps

6. Verify Isolation

Verify that the machine is de-energized

Verify that the machine is isolated

Verify that stored energy has been released

Report Errors to ManagementSlide25

If devices must remain in place after shift is over:Lockout/Tag Out protection must be continuousLockout/Tag Out protection must have an orderly transfer between employees

Device Removal

Report Errors to ManagementSlide26

The lockout or tagout device must be removed by the authorized employee who applied it.The lock-out device may be removed under direction of employer if authorized employee unavailable.

Device Removal

Report Errors to ManagementSlide27

Steps employers must take before removing a lock or tag:Verify that authorized employee who applied lock is not at the facility

Take all reasonable efforts to contact the authorized employeeDevice Removal

Report Errors to ManagementSlide28

If crucial to remove, and employee cannot be reached, call the Plant Manager or the Maintenance Operator.Inspect the work areaEnsure all employees are safeNotify all affected employees
