A copy of the Cameroonian Nati o nal Ide ntity Card if available (copy of front and back); Note: At least o ne of the Cameroonian i d entity documents (Passport or National Identity Card) mu st be U NE XPIRED ! 6. Proof of CNPS M emb ership : copy of CNPS membership card showing the insured number under which the individual receives t heir pension; 7. a . Notarized sworn statements by three ( 0 3) witnesses residing in the same area (city and state) as the app lica nt : The witnesses mus t specify the date when they have physica lly seen the applicant and testify that the applicant is alive . The contac t informatio n (complete address and phone numbe r) of each witness must be included in the s worn statement ; b. P ro of o f i de ntification for each witness (copy of valid ID : state ID, drive r ’ s license, passport, green card ) ; 8. Co pies of i mmigration do cument s indicating the applicant’s residency status in the United St ates : Proof of Residency Status in the United States: - F or perm an ent re s idents: photocopy of the green card; - For applic ants whose ca se is awaiting a final decision , or about to start: pr o vi de co pi es of any available documents justifying that a procedure of adjustment of status is in progress or about to start ; - For int er nation a l students in American schools: copy of the mos t recent form I - 20 , along with a letter from the International Stud e nt s Off ic e of the school confirming a current enrollment under the F - 1 student status ; - For other cases: provide a copy of any av aila bl e immi g ration documents. 9.
