slide2. Objectives Gather baseline information on the knowledge, attitudes and practices of TB patients and the contacts in Indonesia to identify barriers to seeking care;
Identify knowledge gaps, cultural beliefs, or behavioral patterns that may facilitate understanding and action, as well as pose problems or create barriers for TB control efforts;
Identify factors influencing behavior that are not known to most people, reasons for their attitudes, and how and why people practice certain health behaviors;
Explore knowledge, attitudes and practices of healthcare and community workers on contact investigation
Assess general awareness of anti-biotic resistance and health care workers practices in relation to AMR<br>

Characteristics of the provider The successful bidder shall be either a for profit or not for profit institution operating in the field of public health, with proven expertise and experience in designing, conducting assessment/research/study/survey and data analysis, as well as formulating recommendation for health policy and program.
An accreditation (institution) or an on-going accreditation process by a certified accreditation body will be an asset
Previous work with WHO, other international organizations and/or major institutions in the field of public health in designing and implementing assessment/research/study/survey is essential and experience in Tuberculosis area of work will be an asset.<br>

slide6. Key requirements The expected outputs of this survey are as the following:
Prior to implementation of the survey:
Finalized Survey Protocol & Study Instrument (Questionnaire)
Upon completion:
Final report of the project (including results and policy recommendations)
Complete final project dataset (all collected and analyzed data)
Activity and financial report
Presentation for dissemination<br>
slide7. Timelines 1 October 2022- 31 October 2023 (1 year)<br>
slide8. Reporting requirements The project manager of the selected contractor will be expected to provide an updated status in a written format on a weekly basis.
Formal reporting (by VC and in the format of a technical report) is expected upon delivery of each deliverable (see above).
Additional reporting activities may be requested by WHO, or initiated by the project manager on a need basis. final report in format word, pdf and in design should be submitted by 31 October 2023<br>
slide9. Performance monitoring Following two aspects of works will be used for performance monitoring:
Timeliness – compliance with the timeframe indicated in the contract.
Quality – as assessed by the PIC in WHO and NTP.<br>
