Recent Highlights Firearm Safe Storage:
Held events at JBLM, Fairchild AFB, and NAS Whidbey Island for transitioning Service Members with a total of 382 storage devices distributed
Trigger locks for long gun, shotgun and pistol available also available for transitioning Service Members

Donations & Community Grant Fund The account consist of revenues received from the prevent veteran suicide emblem sales, & all receipts from gifts, grants, bequests, devises, or other donations from public and private sources. Funds will go to community organizations that provide evidence-based Suicide Prevention programs to Veterans. 4 $10 each + $8 one-time processing fee For more information on how to purchase your 988 emblem:

SSG Parker Gordon Fox SSG Parker Gordon Fox died by suicide, July 21, 2020 at the age of 25.
He joined the Army in 2014 and was stationed at Caserma Ederle in Vicenza, Italy, before working as a Sniper Instructor at the U.S. Army Infantry School in Ft. Benning, GA. Parker was a talented musician and singer who enjoyed playing his guitar with friends and family. He treasured his 1962 Gibson B-25, gifted to him by his grandfather. Music was an integral part of his life, and he loved discovering new artists and songs. Parker’s vibrant personality left an impact on everyone he encountered, and stories abound of his kindness and generosity towards others in their times of need.<br>

Screening & Eligibility: Columbia Suicide Severity Rating Scale Applicant must score at least a “1” indicating low risk of suicide (Scores range from 0-3)
Applicants scoring a “3” using CSSRS are connected with emergency behavioral health services<br>

Suicide Risk Factors: Psychosocial Evaluation Form Applicants must score at least a “1” indicating at least one challenge they are facing<br>

Screening & Eligibility: Military History Eligible military statuses:
Deployed Active Duty
Activated Reserve/National Guard
Veterans (Any discharge other than Dishonorable)
Military/Veteran families*
Their Veteran/Service Member must already be enrolled within SSG Fox SPGP
Military history is confirmed using DD214, VA Award Letter, SQUARES 2.0 system, etc.<br>

General Self-Efficacy Scale (GSE)
Socio-Economic Status Scale (SES) SSG Fox SPGP collects data using 5 behavioral health forms in a pre-and-post setting in order to track changes, or effects, as a result of their time within the program.<br>

Goal Planning Using a Peer Support approach, SSG Fox SPGP works with participants to create SMART goals in alignment with their needs<br>

Third Party Direct Payments SSG Fox SPGP is authorized to conduct third-party direct payments for enrolled participants. To include assistance with:
Transportation
Child Care
Food Assistance
Housing Assistance
General Suicide Prevention Assistance<br>

Regularly Scheduled Check-ins SSG Fox Staff will conduct regular check ins with participants once their goal plan is developed, and referrals are made.
Frequency is dependent on participants needs and comfort level
Providing Peer Support as needed
SSG Fox staff will conduct service attendance checks with the referred organization
Assessing for best-of-fit with referral, tracking attendance at scheduled appointments
Paid referral organizations collect BH survey data to track changes over time.<br>

Reassessment Following completion of services with referral, SSG Fox Staff conduct reassessments with the participant, to include the CSSRS & Psychosocial forms
If the participant scores a “0” using the CSSRS & Psychosocial form, the participant begins the exit process
If the participant scores a 1 or above, new goals are identified, and referrals made (Circling back to phase 1).<br>

Exiting Steps Participants that score a 0 on both the CSSRS and Psychosocial forms are considered “no longer at risk for suicide”.
All five BH assessments are readministered for final scores.
A satisfaction survey is sent out to the participant digitally.
Monthly check ins are provided if the participant wishes for continuity of care (up to six months).
Following six months, participant is officially unenrolled from services
Participant is then enrolled within the SSG Fox Alumni Network<br>
