(Incident 1000424086)Operation / Project: RTKC Mine Operations – Roads & ProjectsLocation: South East Waste Rock – A4 SectionDate: January 19, 2016Critical Control Absent:Contact: Amit Walia (Amit.Walia@riotinto.com)What happened:At approximately 10:40am, a Mayday was initiated in response to the south east waste rock expansion A4 area. A mine operations employee was in the area conducting an inspection for the future establishment of dump catchment berms. The employee was walking down from the top of a windrow, felt his right knee pop, and fell to the ground. Diagnosis determined that the employee sustained injury to the right knee resulting in a right closed tibia plateau fracture. Immediate actions implemented:Environm en t: Area was secured and preliminary investigation commenced . E ngagement : Safety meetings, with current and oncoming crews, were held with focus on the risks associated with walking and working on uneven surfaces. Interdepartmental evaluation around site exposure for walking on uneven surfaces. People: The employee was assessed by lark EMT and transported off site by county ambulance to the local hospital for further evaluation. Prelimi n a r y causes:Environment: Loose rock, uneven surfaces, ground conditions.Equipment: Proper PPE was utilized (hard hat, safety glasses, good boot condition).People: Employee loses footing on windrow and descends uncontrolled resulting in fracture to right tibia. Initial learnings:Environment: Assess work area conditions and apply needed controls (i.e. clear inspection area)Equipment: Assess if area needs to be cleared by equipment to perform an inspection.People: Firm footing, knowing where to safely stand to mitigate fall hazard. Approximate location where fall occurred.

The switchman suffered severe leg trauma to both legs. The switchman was treated at the scene and flown by helicopter to a local hospital where he is receiving care.Immediate actions implemented:Environme nt: Area was secured and scene was preserved . E ngagement: All rail switching operations have been stopped pending an investigation which is underway. People : E mployee Assistance Program (EAP) grief counsellors have been meeting with impacted team members. Initial learnings:Equipment: OEM procedures for performing this task lack specificityPeople: This is a non-routine task performed less than once per year.Installation area  Impact SiteBroken Come AlongRight #6 tireFallen Cylinder Incident: : Craftsman Injured while trying to avoid falling Haul Truck Hoist Cylinder Preliminary Classification: Lost Day Injury (LTI ) Operation / Project: Kennecott Copper Location: Bingham Canyon Mine Date/Time: 17 February 2016 @ 8:30 a.m.Critical Control: Lifting Plan ExecutionContact : Jake Buzianis (801) 569 6241Preliminary causes:Equipment: A portion of the rigging equipment may have been shock loaded during the lifting process – To be confirmed as part of the investigationWhat happened:At approximately 8:30 am three craftsmen were installing a 2055 lb hoist cylinder on a Komatsu 930-3SE haul truck. During the installation, the cylinder fell approximately six feet to the ground and rebounded when it hit the ground. The injured craftsman was out of the initial line of fire when the cylinder fell. However, he was injured when he was struck by the cylinder when it deflected off of the floor which resulted in him falling backwards to the ground. The craftsman has a bruised left knee from the cylinder impact and injured his lower back in the process.
