X 12 metre length CS pipe from l yard to fabrication shop. . What happened?. A cementing banana HGV was driving on a graded road in the Lekhwair area at night having travelled from Fahud. D. river . did not negotiate the graded road curve and went straight crossing the left edge of the graded road . What . happened?. An instrument crew consist of 06 personnel including driver on their way to base in a 03 ton canter on completion of day’s job , rolled over while negotiating a sharp left bend on a graded road. A Welder & Fabricator were being lifted to their working area in a man riding basket by wheel loader with fork attachment. The wheel loader operator was watching the activity by keeping the forks parallel to the ground. While the activity was ongoing for 1 hr and 45 min., the man riding basket slid off the forks, resulting in a fall from a height of 2.68m to the ground. The welder landed outside of the basket while the fabricator remained inside. Both people sustained minor injuries.. An Auto-mechanic was removing the clutch disc from a Water tanker truck in P&E workshop with the help of his colleague. While doing this activity the clutch disc slipped and fell on his left hand middle finger tip causing a crush injury. He was given First aid at site and from hospital he was diagnosed with fracture in tip of the finger.. title: . Barge incident. What happened?. During . some rough weather one of the two mooring lines holding the maintenance barge to the mooring buoy failed. An assessment by the marine maintenance team resulted in a decision to use a small boat to secure a towing line to the barge. While preparations were underway it was noticed that the barge and the mooring buoy were drifting towards the MAF beach.. title: LTI. What happened?. The task was to prepare nipple down BOP. After unscrewing all the BOP nuts and studs, the Floorman-1 was coming out from the cellar, holding the pad eye of the substructure lower beam. At the same time, the Floor man-2 . What happened?. On the 20/07/2018 at approximately 17:45 hours, . a . crew helper was placing the prime movers wheel chocks back into the storage bracket, located on the passenger side of the prime mover in front of and in close proximity to, the passenger side rear wheels, as the helper entered in to the gap between the 1st rear axle and the fuel tank the driver started moving his prime mover forward, resulting in the vehicle rear wheels striking the helper, causing him to fall to the ground and the vehicle wheels drove over the helpers legs. . The flowline pipe was rested on the tripod and due to the stress relief the pipeline straightened and fallen on the permit holders legs as he was walking parallel to the line which resulted in fracture to his right leg. .
