Person Centered Planning underlies and guides respectful listening which leads to actions, resulting in people who:
Have positive control over the life they desire and find satisfying,
Are recognized and valued for their contributions to their community
Are supported in a web of relationships, both natural and paid, within their communities

Person Centered Planning is a step with the Self-Determination process.
The service recipient is the center and the driver of the process.
Self-Determination is a special funding program of the regional centers where clients have greater control over their budgets and spending plans.

Focuses on gathering information for planning based on the story of a person.Places the individual at the center of the process

Focuses on what is important to someone now and balancing this with things that we need to know or do to keep the person healthy, happy and safe.
Action Plan
Living document that changes and grows with the person.
