New Employee Benefits begin the 1st of the month following a full calendar month worked Examples :  Hire date 7/1/18 = Benefits Effective Date 8/1/18        Hire date 7/25/18 = Benefits Effective Date 9/1/18 Employees  transferring from another Georgia district or State Health covered position / Transfers:    Examples: Transfers receiving last payroll checks with prior district 7/31/18 will receive 1st Bibb check 8/31/18 - Benefits effective 9/1/18                      Transfers receiving last payroll checks with prior district 8/31/18 will receive 1st Bibb check 9/30/18 - Benefits effective 10/1/18Note: Premiums are Paid 1-month prior to coverage effective dateException: Flexible Spending Account premium deductions start the coverage effective month Semi-Monthly Employees : Payroll deductions for benefits will be split between 2 paychecks each month   No changes until Open Enrollment without a Qualifying Life Event Qualifying Life Event changes are allowed within 30 days of the event (i.e. marriage, birth, loss/gain of coverage)Open Enrollment is mid-October through early-November for January 1 coverage Benefits Eligibility / Premium Deductions

Bibb County School DistrictSummary of Benefits Health Plans offered are the same in all Georgia public school districts, whether working in Bibb, Houston or  Monroe, etc. If you are a transfer from another Georgia district, your health election will transfer with you . Changes will be allowed at the next open enrollment.Non-Medical benefits differ from county to county. If you are a transfer from another Georgia district, your non-medical benefits will end with your prior district, and benefits you enroll in with Bibb will begin on your effective date.New hires are eligible for term life insurance policies, but during open enrollment in the fall, employees may also enroll in whole life policies. A Cafeteria Plan is a reimbursement plan governed by IRS section 125, which allows employees to make deductions on a pre-tax basis for the benefits listed. Summary of Benefits

Basic Group Life is provided by the district at no cost to employees. The benefit is 1X your annual salary, rounded up to the nearest $1,000. An equal amount of Accidental Death is included with the benefit.  $10,000 minimum $50,000 maximum Dependent Life  $5,000 Benefit for Spouse & Child(ren) age 14 days to 19 or 26 if a full time student.  A reduced benefit of $500 for Child(ren) age 14 days to 6 months is also available. Medical approval required for family if coverage is not added as a new hire.Supplemental Group Life  Coverage is available on Employees from 1 to 5 times annual earnings rounded to the next $1,000.  Maximum amount is $500,000. Guarantee Issue as a new hire is up to 3X earnings to a maximum of $500,000. Medical approval required if coverage is not added as a new hire.Beneficiary Data Must be listed on the  Non-Medical Benefits/Houze Website for life insurance purposes:List at least 1 primary and 1 secondary beneficiary (both must be at least 18 years old).If you do not enroll in any benefits, you automatically have the board paid group life insurance and must  go online to list beneficiaries for that purpose. Flexible Spending Account / FSA Medcom is the FSA administrator The Flexible Spending Account offers a tax-advantage of paying your out-of-pocket medical expenses and dependent care expenses with pre-tax dollars – all with the convenience of a Mastercard. Look for your debit card by mail after you enroll! Two accounts to choose from:Dependent Care FSA (daycare, after school program fees) - annual max $5,000 per household Use-it-or-lose-it - all funds in account must be used by December 31  Wait until payroll contribution is made for reimbursement Healthcare FSA (medical, dental, vision costs) - annual max $2,600Up to $500 may be rolled over to next year for active employees May use annual contribution on coverage effective date Keep all receipts, even for debit card purchases – documentation may be requestedEmployees do not have to be enrolled in district health plan or other plans to participate IMPORTANT NOTICE: Flexible Spending Benefits are only available to “active” employees. If you leave the district, the amount of money remaining in your flex account is forfeited. Your flex card will become inactive on the month your employment/contract ends. In addition, if you are in a “leave without pay” status, your card will be inactive during the months you are out FSA contributions must be re-elected each year during open enrollment in order to continue contributions during the next calendar year

Disability Plans offer partial income replacement in the event you are ill or injured and unable to work Choose Monthly Disability Benefit: Elect from $200 to 66 2/3% of your monthly salary. The monthly benefit check is issued by Cigna, not the district Choose Plan: Plan 1 (payable to age 65) or Plan 2 (payable for a maximum of 5 years) - both plans work as a short term plan, and convert to a long term plan as needed. The difference is how long the plan would pay in the event of a long term disability.Choose Elimination (waiting) Period: choose from 5 waiting periods: 14, 30, 60, 90 or 180 days - the number of days you must be out of work before you become benefit eligible. You must use sick leave before you receive the disability benefit.NOTE: If you become eligible to draw social security or retirement while receiving disability, the amount of disability will decrease. This could cause requirement of re-payment of funds already received.Maternity Benefits: Paid the same as for an illness.Pre-existing Condition: The plan does not pay a benefit if the disability is due to a pre-existing condition, and you become disabled during the first 12 months coverage is in effect. A pre-existing conditions includes any condition, including pregnancy, for which you received treatment or consultation within 3 months before coverage begins. Medical approval required if coverage is not added as a new hire.Refer to www.houze.org/bibbschools for more details. Aflac Cancer & Off-the-Job Accident Policies are available and pay in addition to your health and disability insurance. Details are available on the website, www.houze.org/bibbschools Aflac policies must be approved and issued. You are required to complete an application and a deduction authorization form to enroll. You may pick up the forms from the Benefits Department. Both policies include an Annual Wellness Benefit 

Mandatory- Non-Medical Benefit Elections All non-medical benefit elections are made via our online electronic enrollment system.  Is your spouse a district employee? If so, avoid duplicate dependent life or dental coverage Step 1: Go to www.houze.org/bibbschools (our benefits website)Step 2: Under New Hire Information click Enroll Step 3: Enter the last 4 of your social as instructedStep 4: Enter your PIN number, which is the first 4 letters of your last name followed by the 2 digit month and 2 digit year of your date of birth.  Example: John Smith, date of birth May 25,1976 would be SMIT0576Step 5: Complete the requested information in the demographic section and make your elections/declinations for each benefit.Note: Even if you do not plan to enroll in any benefits, you still receive the board paid group life insurance and, as such, you must list beneficiaries for this benefit.Step 6: When you have completed your elections, click “Continue ” at the bottom of the enrollment form and re-enter your PIN number as your electronic signature. Blue Cross Blue Shield of GA (BCBS) & United Healthcare (UHC) plan options:Blue Cross Blue Shield Health Reimbursement Arrangements (HRA's): all 3 levels of the HRA plans offer the same coverage, with the following differences: Monthly premium/cost – the amount of employee payroll deductions Bronze - least expensive 72.45-single rate / Gold - most expensive 168.73-single rateBase HRA contribution – dollars placed in account for first dollar use (medical or pharmacy)Bronze offers fewer HRA dollars (contributions), $100 single plan/ Gold offers the most, $400 single planAnnual deductible – yearly amount employee pays out of pocket after HRA dollars are usedBronze = highest deductible $2,500 individual, Gold = lowest deductible $1,500 individualPayout percentage – the amount plan pays after annual deductible is met75% for Bronze, 85% for GoldThese plans offer in and out of network coverage and do not require a co-payment Rx coverage is the same across all 3 levels of the planOnce HRA dollars are used, members pay 100% of cost until deductible is metBlue Cross Blue Shield and United Healthcare HMO’s: Both plan designs are the same. They offer co-payments for visits and Rx's and a lower deductible.  No out of network coverage. United Healthcare HDHP: Plan offers the lowest monthly premium and a high deductible. All services including Rx are subject to deductible.  In and out of network coverage offered.Kaiser Permanente is available for employees who live in the Metro-Atlanta area only. The plan is designed as a “one stop shop” for medical and pharmacy.All plans offer wellness visits (annual physicals), covered at 100% and do not require co-payments at time of service.In addition to the employee rate, the district contributes $945 to each employee’s health plan - $11,340 per year.Health InsuranceHealth Insurance

I Mandatory – Medical Benefit Election Medical benefit elections are made via the SHBP/ADP online enrollment system. Dependent Eligibility:  Your legal spouse, children and step-children (to age 26) are eligible for health insurance.Spouse/dependent documentation is required:Follow specific ADP instructions to add dependents to the medical plan - instructions will arrive by U.S. mail and/or e-mail address on fileSubmit documentation to ADP in the format required by the deadline provided by ADPADP requires dependent social security numbers upon enrollment Failure to submit documents requested by ADP will result in dependent coverage being dropped

Additional Employee Benefit Options Register to view your paycheck, annual W-2 form, leave, etc. Contact the vendor of your choice to start contributing today to 403(b) or 457(b) plans!   

Employee Wellness Center Fitness center for employees and their dependents. Aerobics, fitness equipment, exercise classes, weights, etc. Monthly membership fee of $10.00 per person, paid by payroll deduction. You may enroll at any time.

Marketplace Coverage The district is required by the U.S. Department of Labor to provide the above notice. For further details, visit www.HealthCare.gov .

Confirmation of New Hire Session Attendance Mandatory: Please complete Confirmation of New Hire Session Attendance.  This confirmation acknowledges your offer of benefits, and states that you must complete the online elections between 7/30/18 and 8/03/18 .

Teachers Retirement System All eligible employees are required to be a member of the  Teacher Retirement System of Georgia (TRS) as a condition of employment.You should receive a packet from TRS, which will include your TRS ID. After receiving your packet, please visit www.trsga.com to list your beneficiary information.  Members currently contribute 6% of their gross salary on a before-federal tax basis. The District also makes an additional contribution which helps fund TRS for current and future retirement benefits and is not part of your account, and is not refundable should you terminate TRS membership.  Member contributions are refundable, if employment terminates before you are vested in your retirement system.
