Applicant’s First Name:
Applicant’s Last Name:
Applicant’s Email:
Department Contact Name:
Department Contact Email:
Purpose of request: (volunteer, student employment, graduate employment, temp classified hire)
*Note: If you are submitting multiple requests, you can submit one email with the necessary information for all applicants.
Slide21The applicants will receive an email from Risk Mitigation Services with a secure link to complete their online authorization.
Slide22The department contact listed on the request will receive the following email when the background check has cleared:

Slide23If a background check reveals any convictions (findings), HR will coordinate with the respective VP to obtain the required approval.
Once VP approval is received, the department contact will receive the automated “clear” email.
If a VP elects to decline the request, HR will contact the applicant and notify the department contact.
