Dental Assistant Application

Fall 2020 Cohorts

Overview of Curriculum

You will apply to a specific campus and will complete all of your courses on that campus Cohort model- all go through program at same time

Program starts in the fall and will finish the following summer

Pre-Dental Assistant Requirements

Students are required to have a HS diploma or GED to be eligible for the Dental Assistant program

Pre-Dental Assistant Requirements

You can apply now, even if you are still working on the requirements Indicate on your application when you are registered for the courses needed

Spring and Summer registration are open Students who have all requirements complete upon application will be given priority registration

If you have courses in-progress, you will be conditionally accepted

If you are unsuccessful in any of the required courses, you will be dropped from the fall Dental courses

Degree vs. Diploma

Diploma is 49 credits AAS is 60 credits Students take the following courses in addition:

Ethics or Medical Ethics 8 credits of any General Education courses

If you are transferring in credits from another school, degree could be a good option or if you’re finished with pre-requisites and need some classes for the spring/summerMake sure to check your Degree Audit Report (DARS) to verify your courses have transferred in to HTC

Immunization Form

Immunization form needs to be completed by an MD, NP, or PA Will not accept clinic record print-outs

Please note that some immunizations can take several months if you do not have them All students need at least the first 2 shots of the Hepatitis B Vaccination to start the program

Tetanus vaccination (TDAP) must be within the last ten (10) yearsIf you do not have your immunization history, your doctor will need to do a titer to check for immunity

Background Study

Within your first two weeks of the program there will be a background check If you are concerned of any issues that may come up in your background study, we advise that you connect with the MN Department of Human Services now to get ahead of any possible issues

Complete the form in blue or black ink and please write legibly Make a copy before you submit to usYou can submit the envelope to Enrollment Services at either campus

Please allow 10 business days to hear back from me with an email about your application status and next stepsIf you are accepted, once fall registration is open you will be able to register for your Dental Assistant courses
